I'm a new patient, what do I need to know?
Prior to an appointment, you (a new patient) will be asked to verify your insurance information.
For your first visit, you'll need to fill out patient information forms and provide us with your insurance identification card(s) so we can photocopy them for our billing records. Forms are available online and in our office.
Providing us with accurate information during your first visit helps facilitate timely filing of claims. Please inform us if you are covered by more than one insurance company and confirm which company is the primary carrier.
